Below are 10 major SEO mistakes made by millions of websites and some advice on how you can avoid them!
01 - Same title on every page
Having duplicate page titles for every page of your website is very bad for your search engine optimization.
Page titles are one of the key on-page SEO ranking factors. They are displayed at the top of your browser window, as well as in the SERPs.

You must define a unique title for every individual page, so that search engines can easily determine which page is which.
02 - Same meta description on every page
Meta descriptions must be different for every individual page. They appear below the page titles in the SERPs.

Your meta descriptions should represent the content of your pages and include your keywords (but not simply keywords). They need to contain around 25 words.
03- No single <H1> tag on every page
Having multiple <H1> tags per page is not good for your SEO. You should not use <H1> tag more than once on a single page.

If you want to have several headings, you need to use subheadings such as <H2> and <H3> tags.
04- No URL rewriting
Many websites have dynamic URLs (URLs containing symbols such as ?, &, $, % and +) that do not attract much search engines (and users).
In order to have more SEO-friendly URLs, you need to get rid of these symbols by rewriting your URLs.

For example, here is a dynamic URL: http://www.example.com/cgi-bin/gen.pl?id=8&view=basic. You could rewrite this URL as http://www.exemple.com/8/basic.html.
Including your keywords in your URLs is also highly recommended for search engine optimization purposes: they will therefore be associated to your pages.

05- Keyword stuffing
Keyword stuffing consists of overfilling your content with your targeted keywords.
This technique used to work in the past, but search engines can now detect unnaturally high incidences of same word or expression. And keyword density is not actually a key on-site SEO ranking factor anymore.

However, avoid keyword density over 5%, otherwise your web page may be penalized by search engines.
06- Home page with only images
Having only images on your website home page is not a good idea for your search engine optimization. Search engines clearly prefer text over images.

07- Internal linking without keywords
One of the worst things you can do for your internal linking is to forget keywords as anchor texts!
It is very important to use keywords in your internal links to the targeted pages, so search engines can associate them with specific keywords.

Do not use generic keywords such as “click here” or large group of words as links!

Another common mistake is to forget to include a title on a header logo. Leaving a logo title empty does not really help search engines know what your image is about…

09- No regular updates on the website
Search engines, as well as visitors, like fresh content. If you do not provide any fresh content, your site will be crawled less frequently.

On the contrary, if you write news or blog posts for example, your content will be searchable more quickly.
10- Using flash
Flash can make a website very beautiful, but it is not very SEO-friendly. Search engines spiders cannot read flash content, and therefore cannot index it.

Flash and search engine optimization are not best friends...
If you really want your website to be Flash-based, it is important to provide an HTML version.
